Key Responsibilities

• Conduct assessments of adult care needs in accordance with legal frameworks and organisational policies.
• Develop, implement, and review person-centred support plans in collaboration with service users and their families.
• Liaise with a range of health and social care professionals to coordinate service delivery.
• Maintain accurate and confidential case records, ensuring compliance with data protection regulations.
• Provide information, advice, and support to adults and their carers regarding available services.

Requirements

• Recognised professional qualification in Social Work, NVQ Level 4/5 in Health and Social Care, or equivalent.
• Proven experience in adult social care assessment and care planning.
• Strong knowledge of relevant legislation, policies, and best practices in adult social care.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to manage a caseload effectively and work flexibly, including some remote administrative duties and community visits in the Stoke-on-Trent region.
