Adult Social Worker: Discharge & Community Care (Fixed Term)

Wirral Metropolitan Borough Council is seeking a dedicated Social Worker to join our team on a fixed-term maternity cover, split between Clatterbridge Hospital CICC and Arrowe Park Hospital/Clatterbridge Hospital. You will deliver personalised, strengths-based support for adults and carers across the borough, working within a multidisciplinary team to promote safe discharges and positive outcomes.
