Your key responsibilities will include:

• Conducting assessments of patients’ social, emotional, and practical needs to develop tailored care plans.
• Collaborating with multidisciplinary teams to ensure holistic and effective patient care.
• Providing information and advice to patients and their families regarding available support services.
• Facilitating discharge planning and coordinating follow-up services to ensure a smooth transition from hospital to home.
