Job Description
Experienced Adult Social Worker – Review Team
We're looking for an experienced Adult Social Worker to join a busy and supportive Adult Social Care Review Team.
Key Responsibilities:
Complete strengths-based Care Act assessments, reviews and support planning.
Manage complex cases, including safeguarding enquiries under the Care Act 2014.
Undertake Best Interest Assessments and support Deprivation of Liberty processes.
Work closely with health professionals and the wider Multi-Disciplinary Team (MDT) to prevent hospital admissions and promote independence.
Participate in the team's duty function, including daily case triage and huddles.
Chair multi-agency meetings and contribute to safeguarding, MASH, MARAC and MAPPA processes where required.
Maintain accurate case records and work in line with Social Work England standards and the Professional Capabilities Framework (PCF).
What We're Looking For:
Qualified Social Worker with Social Work England registration.
Previous experience within Adult Social Care, including complex assessments and safeguarding.
Strong knowledge of the Care Act 2014, Mental Capacity Act and Best Interest processes.
Excellent communication skills and the ability to work effectively within a fast-paced multidisciplinary environment
