Job Title: Adults Advanced Practitioner within Hospital Discharge in Salford (UK)
Salary: £32.55 per hour
Contract Length: 12-month contract

We have an exciting opportunity for an Adults Advanced Practitioner to join the Hospital Discharge team at Salford Royal. This role offers a 12-month contract paying £32.55 hourly, operating Monday to Friday from 08:00 to 16:00. You will play a critical role in supporting hospital discharge processes, covering maternity leave with consistency and commitment that sets this position apart from typical locum roles.

What you will do:

• Coordinate and facilitate the discharge process for adults, ensuring safe and efficient patient transitions
• Work closely with multidisciplinary teams to assess and address complex discharge needs
• Provide expertise and support to less experienced staff handling challenging cases
• Liaise with community services to ensure comprehensive post-discharge care plans
• Maintain accurate records and reports as required by regulatory standards and hospital policies

Requirements of the Advanced Practitioner:

• Degree or equivalent in social work
• Current registration with Social Work England
• Significant frontline experience in Social Services within the UK, particularly in a hospital setting
