Adults Social Worker – Integrated Network Team

Bridgend Council is looking for anexperienced Adults Social Worker to join their East and West Integrated Network Teams on initial 3 month contracts. Full-time candidate required but happy to receive applications from those considering part time. The East is based at Glanrhyd Hospital, Bridgend.

You will be working as part of a well-established multi-disciplinary team supporting adults aged 18+ with frailty, long-term conditions, disabilities and often additional mental health or emotional wellbeing needs.

The role:

• Adults community social work within an Integrated Network Team

• Managing complex cases with a strengths-based, person-centred approach

• Care coordination and multi-agency working

• Regular visits required (driver essential)

• Hybrid working - 2 days from home, 3 days on site

What we’re looking for:

• Qualified Social Worker withpost-qualifying adults experience

• Registered with Social Care Wales

• Confident working within an MDT environment

• Strong assessment, care planning and care coordination skills

• Able to work autonomously while contributing to a wider team

To apply for this role, you will need to hold -

• A minimum of 6 months experience of working in the UK as a registered social worker
• A relevant Social Work Qualification
• Registration with Social Care Wales / Social Work England
• Enhanced DBS, registered on the update service or be happy to apply for a DBS
• 3 Years of referencing
• Driving license with access to a car
