Location: Manchester Council: Manchester City Council Contract: Temporary / Full-time

Tripod Partners are currently recruiting for an experienced Adults Social Worker to join the Adult Early Support Team at Manchester City Council.

This is an exciting opportunity to join a team focused on providing early intervention and support to adults, helping people maximise their independence and achieve positive outcomes.

The Role

You will:

• Manage a caseload of adults requiring support

• Complete Care Act assessments and reviews

• Identify individuals' strengths, needs and desired outcomes

• Develop person-centred support plans

• Promote independence and prevent escalation of need

• Work collaboratively with individuals, families and carers

• Liaise with health and other partner agencies

• Assess and manage risk

• Undertake safeguarding responsibilities where required

• Maintain accurate case records and documentation

About You

The ideal candidate will have:

• A recognised Social Work qualification

• Current Social Work England registration

• Experience working within Adult Social Care

• Strong assessment and care planning experience

• A person-centred approach

• Excellent communication and organisational skills

• The ability to work effectively within a multi-disciplinary environment
