This is an excellent opportunity to use your advanced clinical assessment, diagnostic and prescribing skills to deliver high-quality patient care while helping shape the future of primary care.

You’ll independently assess, diagnose and manage patients presenting with a wide range of primary care needs, making autonomous clinical decisions and providing safe, evidence-based care. Working alongside experienced GPs, Practice Nurses, Wellbeing Advisors, Clinical Coders and other healthcare professionals, you’ll play a key role in delivering outstanding care to our diverse patient population.

Main Responsibilities

As an Advanced Clinical Practitioner, you will:

• Independently assess, diagnose, treat and manage patients within your scope of practice.
• Make autonomous clinical decisions using advanced assessment and clinical reasoning skills.
• Prescribe safely and effectively as an Independent Prescriber.
• Deliver high-quality, patient-centred care within a multidisciplinary team.
• Contribute to clinical leadership, governance and the continuous development of services.
• Support colleagues through collaborative working and the sharing of knowledge and expertise.
• Help drive innovation and service improvement within the practice.
