Job overview

We are seeking an Advanced Clinical Practitioner, who has a passion for admission prevention to join a core Team of ACPs to deliver a countywide Hospital at Home ( ) in Worcestershire. The service is integrating with Respiratory VW so strong experience and knowledge of managing respiratory cases is required for this role to support the development of the service.

This is an exciting opportunity to be part of the delivery of a developing service and to play a significant part in shaping this service. There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders, in particular with Acute Trust colleagues.

The service is countywide across Worcestershire so the ability travel across the county will be expected. The service is operational 8am-8pm seven days a week including Bank Holidays so the post holder will be expected to work shift patterns to enable cover of these operational hours including regular weekend and Bank Holiday working. Full and Part Time hours will be considered.

As an experienced practitioner you will have completed the Advanced Assessment Skills and Non-Medical Prescribing (V300) modules holding the full masters qualification.

Main duties of the job

There will be the requirement to assess and manage acutely unwell patients in the Community who otherwise would have been in an Acute Hospital setting with the support of a dedicated medical model for the Hospital @ Home.

The ability to work independently is essential as the post holder will need to offer urgent care, treat minor illness, give patient advice and offer chronic disease management. You will have the ability to prioritise and triage the needs of patients, making any referrals for further investigations where appropriate in line with local and practice guidelines, whilst ensuring your clinical practice is safe and effective.

The postholder will lead in the management of the respiratory caseload and the supervision and education of the rest of the Team in the management of this caseload of patients.

Detailed job description and main responsibilities

As an advanced Clinical Practitioner, you will work with a high degree of autonomy, demonstrating core capabilities in clinical competency, encompassing the four pillars of clinical practice. You will treat patients with acute exacerbations of chronic diseases, manage long term conditions and assess, plan, and treat

patients to prevent them being admitted to hospital . You should have excellent clinical, written, and verbal communication skills, be able to work independently and manage your own workload. For purposes of the role, you must be NMC registered, have good IT skills and have excellent communication skills.

Qualifications

• Registered Nurse Level 1 / Allied Health Professional / Paramedic / Pharmacist
• MSc Advanced Clinical Practice
• Non-Medical Prescribing (V300) / ability to work within PGDs, within scope of professional practice
• Advanced Assessment Skills Module
• Evidence of a portfolio of knowledge, skills and professional development

Experience

• Demonstrable post-graduate experience in a relevant area
• Extensive post-graduate experience in a role undertaking assessments of complex needs
• Experience of working with people with long-term illness and / or older people
• Clinical experience of caring for people with cognitive impairment
• Experience of working across professional boundaries
• Experience of change/leading service development
• Experience of clinical leadership
