Responsibilities

In this autonomous, clinically advanced role, the Advanced Practitioner will:

• Assess, diagnose, treat and refer patients with a wide range of conditions.
• Lead on safe, effective, evidence‑based care in a triage setting.
• Conduct and interpret diagnostic investigations.
• Liaise with specialist services and ensure appropriate handovers/referrals.
• Maintain meticulous records in line with governance and best practice.
• Stay at the forefront of evidence‑based practice and CPD.
• Practice autonomously without direct supervision.
• Maintain appropriate clinical records and comply with current practice guidelines.
• Work effectively as part of a multi‑professional team in collaboration with other members of the service.
• Adhere to performance measures, infection prevention and control policy, safeguarding procedures, and relevant legislation.
• Prescribe medication within the limits of extended non‑medical prescribing law; handle ordering, storage, administration and disposal of medicines.

Managerial duties include:

• Participate in clinical audits, team meetings and professional meetings.
• Support implementation of policies and provide guidance to staff.
• Lead and participate in the clinical audit cycle.
• Coordinate staff supervision and delegation.
• Participate in appraisal processes and identify performance issues.
• Ensure efficient use of material resources and identify opportunities for improvement.

Clinical governance responsibilities:

• Participate in governance requirements, quality improvement and research and development activities.
• Provide mentorship, leadership and training to other team members.
• Assess outcomes of own practice and report shortfalls to managers.

Education and professional development:

• Participate in the clinical supervision framework and ongoing professional development.
• Mentor and role‑model in triage consultation and junior staff training.
• Complete mandatory training programmes.

Qualifications

• Registered Adult Nurse with the Nursing & Midwifery Council (NMC), Paramedic registered with the Health Care Professions Council (HCPC) or Pharmacist registered with the General Pharmaceutical Council (GPhC) and registered as an independent non‑medical prescriber.
• Minimum of 2 Level‑7 modules, including a module on assessment of clinical examination skills.

Experience

• Substantial autonomous practice in a multi‑professional team.
• Evidence of autonomous physical examination and assessment skills.
• Broad knowledge of complex and common conditions in primary care for adults and children.
• Experience in nurse‑led clinics, emergency care centres, out‑of‑hours facilities or walk‑in centres.
• Consolidated independent prescribing experience.

Knowledge and Skills

• Ability to complete concise clinical history and physical examination.
• Telephone triage experience.
• Formulate differential diagnoses and management plans.
• Effective communication and negotiation skills.
• Excellent verbal and written skills.
• Critical thinking and time‑management.
• Commitment to lifelong learning and continuing professional development.
• Team‑player attitude and ability to work autonomously.
• Willingness to undertake additional training to achieve a full MSc in Advanced Clinical Practice.

• High level of interpersonal skills, confidence, compassion and clarity under pressure.
• Evidence of independent prescribing, autonomous decision making and leadership in clinical care.
• Commitment to patient safety, clinical governance and infection prevention/control.
• Ability to work effectively with a diverse workforce and within the parameters of regulatory codes.
