Advanced Practitioner (Physio/OT) – Clinical Lead, Pathways

East Sussex Healthcare NHS Trust is recruiting a HCPC-registered Physiotherapist or Occupational Therapist to lead advanced clinical practice across acute and community settings. You will collaborate with MDT teams to redesign pathways, support discharge, and champion patient and family involvement in care planning.

The role requires experience in acute, community, IDT or integrated care, strong leadership and QI skills, and a commitment to Home First with flexible working.
