Band 6 - Inpatient Older Adult Discharge Social Worker

Day Webster are currently recruiting for a Band 6 Inpatient Older Adult Discharge Social Worker within Acute Mental Health Services, ensuring effective patient flow and optimal use of inpatient beds.

You will be required to work collaboratively with multidisciplinary teams, community services, primary care, partner agencies, and families to identify and overcome barriers to discharge from admission onwards. Provide specialist knowledge of social care, housing pathways, Mental Health Act (s117), Mental Capacity Act, and No Recourse to Public Funds legislation. Support carers, maintain strong links with statutory and voluntary organisations, supervise junior staff, and contribute to reducing length of stay while promoting person-centred outcomes for older adults and individuals requiring complex discharge planning. What you need: Essential

The applicant must have a minimum of two years post graduate Social Work experience. The applicant must have two years post qualification experience in a mental health setting.
