Care Co-ordinator, Community Mental Health (Flexible Working)

Pennine Care NHS Foundation Trust is seeking a Care Co-ordinator to assess, treat, and deliver care for adults with complex mental health needs in the community. You will ensure high-quality care aligned with trust policies and regulatory requirements.

You will conduct holistic mental health and risk assessments, manage a diverse caseload, deliver evidence-based interventions, and participate in multidisciplinary meetings while supporting student nurses.
