Care Coordinator (Domiciliary Care)

About the Role

Our client is looking for an organised and diligent Care Coordinator to manage domiciliary care services in Oxford . This essential role involves scheduling care visits, managing a team of care workers, and ensuring the delivery of high-quality, personalised care to clients in their own homes. You will be responsible for liaising with clients and their families, assessing needs, and ensuring compliance with regulatory standards. This is a critical position for someone passionate about facilitating independent living for individuals in the Oxfordshire area.

Key Responsibilities

• Develop, manage, and update client care schedules, ensuring optimal coverage and efficient routing of care workers.
• Recruit, train, supervise, and support a team of domiciliary care workers.
• Conduct client needs assessments and develop appropriate, person-centred care plans.
• Liaise effectively with clients, their families, and healthcare professionals to ensure satisfaction and coordinated care.
• Monitor the quality of care delivery, responding promptly to any concerns or issues raised.
• Maintain accurate client records, care logs, and staff documentation in compliance with CQC and company policies.

Requirements

• Proven experience in a Care Coordinator or management role within domiciliary care.
• Strong understanding of CQC regulations and best practices in home care.
• Excellent organisational, planning, and time-management skills.
• Effective communication and interpersonal skills, with the ability to build rapport with clients and staff.
• Proficiency in care management software and general IT systems.
• A relevant qualification such as a Level 3 or 5 Diploma in Health and Social Care is highly preferred.
