About the Role
Our client is looking for an experienced and compassionate Care Coordinator to oversee and manage support services for elderly individuals in Southampton . This vital role involves assessing needs, developing care plans, and coordinating a team of care professionals to ensure the highest standard of care and well-being for clients. The position offers a hybrid working model, blending essential field visits with remote administrative tasks, providing flexibility while ensuring comprehensive service delivery within the Hampshire region.
Key Responsibilities
- Conduct comprehensive assessments of elderly clients' needs, including physical, social, and emotional well-being.
- Develop, implement, and regularly review personalised care plans in collaboration with clients and their families.
- Coordinate the scheduling and deployment of care staff, ensuring appropriate matches and service continuity.
- Act as the primary point of contact for clients, their families, and external healthcare professionals.
- Monitor service quality and client satisfaction, addressing any concerns or issues promptly.
- Maintain accurate client records and prepare regular reports on service delivery.
Requirements
- Previous experience in care coordination, case management, or a similar role within elderly care.
- Strong understanding of the needs of older adults and best practices in geriatric care.
- Excellent communication, interpersonal, and organisational skills.
- Ability to manage a diverse caseload and prioritise effectively.
- Proficiency in using care management software and standard office applications.
- A relevant qualification in Health and Social Care is highly desirable.
