Care Coordinator (Home Based)

About the Role

Our client is seeking a highly organised and empathetic Care Coordinator to manage and oversee care packages for individuals within the Milton Keynes area. This role is pivotal in ensuring seamless delivery of high-quality support services, coordinating between clients, their families, carers, and healthcare professionals. You will be responsible for the operational aspects of care delivery, including scheduling, resource allocation, and quality assurance, directly impacting the well-being and independence of those we serve. This is an essential, hands-on role requiring strong logistical and communication skills.

Key Responsibilities

• Develop, implement, and review personalized care plans for clients.
• Schedule and manage the rotas for care staff, ensuring adequate coverage and client needs are met.
• Act as the primary point of contact for clients, families, and care staff, addressing queries and concerns promptly.
• Conduct initial assessments and regular reviews of client needs and care provision effectiveness.
• Ensure compliance with all regulatory requirements and company policies, maintaining high standards of care.
• Liaise with external health and social care professionals to ensure integrated support.

Requirements

• Previous experience in a care coordination or similar management role within the social care sector.
• Thorough understanding of care planning, assessment, and CQC regulations.
• Excellent organisational and time-management skills, with the ability to multitask effectively.
• Strong communication and interpersonal skills, capable of building positive relationships with diverse stakeholders.
• Proficiency in relevant software for care management and scheduling.
• Demonstrated ability to handle sensitive information with discretion and professionalism.
