About the Role
Our client is seeking a diligent and organised Care Coordinator to join their team in Birmingham . This role is central to ensuring the seamless delivery of community-based care services to a diverse client group. You will be responsible for scheduling care visits, managing care staff rotas, and liaising between clients, their families, and the care team to ensure consistent and high-quality support. This hybrid position requires a blend of office-based strategic planning and client-facing interaction, offering an excellent opportunity to contribute to the effective operation of a crucial community service in the West Midlands . You will play a key role in maintaining service user satisfaction and operational efficiency.
Key Responsibilities
- Develop and manage detailed weekly schedules for the care team, ensuring all client needs are met.
- Conduct initial client assessments and contribute to the creation of bespoke care plans.
- Effectively communicate with clients, their families, and care staff to address queries and resolve issues promptly.
- Maintain accurate and up-to-date client and staff records using company software.
- Monitor service delivery, identifying any gaps or areas for improvement in coordination.
- Ensure compliance with all regulatory requirements and company policies.
Requirements
- Previous experience in a care coordination or scheduling role within the social care sector.
- Strong organizational and time-management skills, with the ability to multitask effectively.
- Excellent communication and interpersonal abilities, both written and verbal.
- Proficiency in using scheduling software and standard office applications.
