We are working with an established and growing Home Care provider in Woking, Surrey , who is looking for a caring, organised and motivated individual to join their team as a Care Coordinator .

This is a fantastic opportunity for someone who understands care from the front line and is ready to take on more responsibility, develop professionally and build a career beyond hands-on care.

If you've had some involvement with staff rotas, scheduling, coordinating visits or arranging cover using ACCESS PEOPLE PLANNER, PASS, CARE BERRY.. etc... , that's a great foundation — and you'll have the opportunity to build on this with training and support.

What You'll Be Doing

Working alongside the Registered Care Manager, you'll help coordinate the day-to-day delivery of care, including:

• Assisting with preparing rotas and scheduling, including arranging cover when required
• Helping to ensure the well being of both clients and care staff
• Supporting safeguarding procedures
• Building positive relationships with clients, families and care workers
• Liaising with Social Workers, GP practices, NHS Discharge Teams and other professionals
• Support the Registered Care Manager with administration and day-to-day coordination
• Providing personal care in the community when required
• Participating in an additional paid weekday & 1-in-4 weekend on-call rota
