About the Role

Our client is seeking a highly organised and proactive Care Coordinator to join their Community & Social Care team in Glasgow . This role is central to ensuring that individuals receive seamless and appropriate care and support services tailored to their specific needs. You will be responsible for assessing client requirements, developing personalised care plans, and liaising with a network of service providers to ensure effective delivery of support. This position requires excellent communication and organisational skills, with a strong focus on client well-being and satisfaction, working within a dedicated local team.

Key Responsibilities

• Conduct thorough assessments of client needs, preferences, and circumstances to inform care planning.
• Develop comprehensive, person-centred care plans in collaboration with clients, their families, and relevant professionals.
• Source, coordinate, and monitor the delivery of a range of care and support services, ensuring quality and effectiveness.
• Act as a primary point of contact for clients and their families, addressing queries and resolving issues promptly.
• Maintain accurate and up-to-date client records, including care plans, progress notes, and service agreements.
• Collaborate with internal teams and external stakeholders to ensure integrated and holistic support for clients.

Requirements

• Previous experience in a care coordination, social work, or similar support role.
• Strong understanding of the care sector, including available services and relevant regulations.
• Excellent assessment, planning, and organisational skills, with meticulous attention to detail.
• Proficiency in using case management software and standard office applications.
• Effective communication and interpersonal skills, with the ability to build positive relationships.
• Ability to work independently, manage time effectively, and prioritise tasks in a demanding environment.
