About the Role
Our client is seeking an organised and proactive Care Coordinator to manage and oversee the provision of high-quality care services in the Southampton area. This pivotal role involves liaising between clients, their families, care staff, and external agencies to ensure seamless delivery of personalised care plans. You will be responsible for scheduling staff, managing rotas, conducting care assessments, and ensuring compliance with all regulatory standards. Key Responsibilities
- Develop, implement, and review personalised care plans for clients, conducting initial assessments and regular reviews.
- Create and manage staff rotas, ensuring adequate cover and efficient deployment of care workers across the designated area.
- Act as the primary point of contact for clients, their families, and care staff, addressing queries and resolving issues promptly.
- Ensure all care services are delivered in compliance with CQC regulations, company policies, and best practice guidelines.
- Monitor the quality of care provided, conducting spot checks and gathering feedback to drive continuous improvement.
- Maintain accurate client and staff records, including assessments, care plans, and attendance logs.
Requirements
- Proven experience in a care coordination , management, or supervisory role within the social care sector.
- Strong understanding of CQC regulations and quality standards in domiciliary care.
- Excellent organisational and time-management skills, with the ability to prioritise effectively.
- Proficiency in using care management software and standard office applications.
- Strong leadership and communication skills, with the ability to motivate and support a care team.
- A flexible approach to work, with the ability to adapt to changing needs and demands in Southampton .
