About the Role
Our client is seeking a meticulous and organised Care Coordinator to join their expanding team in Stoke-on-Trent . This role is central to ensuring the seamless delivery of care services to clients, involving the scheduling of care visits, matching clients with suitable carers, and liaising between clients, families, and the care team. You will play a key role in maintaining high standards of care delivery and ensuring client satisfaction. This position offers a blend of office-based administrative tasks and the opportunity to connect with clients and staff, contributing to a supportive and efficient care operation.
Key Responsibilities
- Develop and manage rotas for care staff, ensuring appropriate coverage across all shifts.
- Conduct client assessments and develop personalised care plans in collaboration with clients and families.
- Effectively match clients with carers based on needs, personality, and skills.
- Act as the primary point of contact for clients, families, and care staff, addressing queries and concerns promptly.
- Maintain accurate client records and ensure compliance with all relevant regulations.
- Monitor the quality of care delivery and implement improvements where necessary.
Requirements
- Previous experience in care coordination, scheduling, or a similar role within the care sector.
- Strong organisational and time-management skills.
- Excellent communication and interpersonal abilities.
- Proficiency in using scheduling software and Microsoft Office Suite.
- Ability to work under pressure and manage multiple priorities effectively.
