About the Role
Our client is seeking a dedicated Clinical Pharmacist to enhance patient care and medication management services in Bristol . This hybrid role will involve both on-site hospital duties and remote responsibilities, including medication reviews and patient consultations. You will work collaboratively with healthcare teams to optimize drug therapy, ensure patient safety, and contribute to the development of clinical pharmacy services. This position is ideal for a pharmacist with a passion for direct patient care and a desire to advance clinical pharmacy practice within a hospital setting.
Key Responsibilities
- Conduct comprehensive medication reviews and provide pharmaceutical care to diverse patient populations.
- Collaborate with physicians and other healthcare professionals to optimize medication regimens.
- Educate patients and their families on medication use, adherence, and potential side effects.
- Monitor and manage drug therapy for chronic and acute conditions.
- Participate in multidisciplinary rounds and contribute to treatment planning.
- Assist in the development and implementation of clinical pharmacy initiatives and protocols.
Requirements
- Registered Pharmacist with a current license to practice in the UK.
- Master's degree in Pharmacy or equivalent clinical pharmacy qualification.
- Proven experience as a Clinical Pharmacist , preferably within a hospital setting.
- Strong knowledge of pharmacotherapy, drug interactions, and evidence-based medicine.
- Excellent communication, interpersonal, and patient counseling skills.
- Ability to manage a hybrid workload, balancing on-site responsibilities with remote tasks.
