About the Role
Our client is seeking a dedicated Clinical Pharmacist to join their progressive healthcare team in Portsmouth . This role focuses on optimizing medication use within patient care settings, collaborating with physicians and other healthcare professionals to ensure safe, effective, and economical drug therapy. You will be involved in medication reconciliation, patient education, and providing expert pharmaceutical advice. This is an excellent opportunity for a clinically focused pharmacist to enhance patient outcomes and contribute to a high standard of pharmaceutical care within a busy hospital or clinic environment.
Key Responsibilities
- Conduct medication reviews and provide recommendations to physicians to optimize patient drug therapy.
- Educate patients and caregivers on medication use, potential side effects, and adherence strategies.
- Participate in multidisciplinary rounds to provide drug information and clinical support.
- Monitor drug therapy for effectiveness, safety, and potential interactions.
- Develop and implement medication management protocols and policies.
Requirements
- Master of Pharmacy (MPharm) degree or equivalent.
- Registered Pharmacist with the relevant professional body (e.g., GPhC).
- Significant experience (3+ years) in a clinical pharmacy setting, preferably hospital-based.
- Strong understanding of pharmacology and therapeutics.
- Excellent communication, interpersonal, and problem-solving skills.
