Key Responsibilities
- Conduct comprehensive needs assessments for individuals requiring social care support in their homes.
- Develop, implement, and regularly review personalised care plans in collaboration with clients and their families.
- Coordinate the scheduling and delivery of care services, ensuring efficient resource allocation.
- Liaise with healthcare professionals, social workers, and other stakeholders to ensure integrated care.
- Maintain accurate and up-to-date client records and care documentation.
- Provide guidance and support to care staff, addressing any concerns and ensuring quality of service delivery.
Requirements
- Experience in social care, healthcare coordination, or a similar role, preferably within the Cardiff area.
- A relevant qualification in health and social care, or demonstrable equivalent experience.
- Strong understanding of care planning processes and regulatory requirements for social care services.
- Excellent organisational and time-management skills, with the ability to manage a hybrid workload effectively.
- Proficiency in using IT systems for record-keeping and communication.
