Key Responsibilities

Requirements

• Experience in social care, healthcare coordination, or a similar role, preferably within the Cardiff area.
• A relevant qualification in health and social care, or demonstrable equivalent experience.
• Strong understanding of care planning processes and regulatory requirements for social care services.
• Excellent organisational and time-management skills, with the ability to manage a hybrid workload effectively.
