About the Role

Our client is seeking an organised and proactive Community Care Coordinator to join their vital services team in Edinburgh . You will be responsible for coordinating care packages and support services for vulnerable adults, ensuring they receive personalised and effective assistance to live independently in their own homes. This role requires excellent communication and organisational skills to liaise between clients, their families, and care providers. You will play a key part in ensuring the seamless delivery of high-quality care within the community, contributing to the well-being of residents across Scotland .

Key Responsibilities

• Assess the care needs of individuals and develop tailored care plans.
• Coordinate the scheduling and delivery of support services from a range of care providers.

Requirements
