Key Responsibilities

• Conduct outreach activities in various community settings to raise awareness about health services and promote healthy lifestyles.
• Engage with individuals and groups to assess their health needs and provide relevant information and signposting.
• Assist clients in accessing healthcare appointments, screenings, and other health-related services.
• Develop and deliver health education sessions and workshops tailored to specific community needs.
• Collaborate with local health providers, charities, and community organizations to build strong referral pathways.
• Utilize digital tools for remote follow-up, information dissemination, and reporting.

Requirements

• Experience in a community-based role, health promotion, or social care setting.
• A strong understanding of public health principles and local health services in the Bristol area.
• Excellent interpersonal and communication skills, with the ability to engage effectively with diverse communities.
• Ability to work independently and as part of a team, managing your own schedule for field and remote work.
• Proficiency in using IT for communication, data collection, and reporting.
• A full UK driving license and access to a vehicle is an advantage for this role.
