Community Navigator

About the Role

Our client is seeking an enthusiastic Community Navigator to join their team in Norwich . This role is designed to connect individuals needing support with the appropriate services and resources within their local community. You will act as a vital link, assessing needs, providing information, and empowering individuals to access the help they require. This hybrid role involves both face-to-face engagement within the Norfolk community and remote administrative duties, contributing to the efficient delivery of Community & Social Care services. The ideal candidate is proactive, an excellent communicator, and passionate about supporting vulnerable populations.

Key Responsibilities

• Conduct initial assessments to identify individuals' needs and support requirements.
• Provide information and guidance on available local services and resources.
• Support individuals in navigating complex systems and accessing appropriate care and social support.
• Maintain accurate and up-to-date records of client interactions and progress.
• Build and maintain strong relationships with local service providers and community organisations.
• Assist clients in developing personal action plans to achieve their goals.

Requirements

• Experience in a similar role within social care, community development, or a related field.
• Excellent understanding of local services and support networks in the Norwich area.
• Strong communication, interpersonal, and active listening skills.
• Ability to work independently, manage a caseload, and maintain accurate records.
• Proficiency in relevant IT systems and databases.
