About the Role
Our client is looking for a proactive and resourceful Community Navigator to join their team in Wolverhampton . In this vital role, you will act as a bridge, connecting individuals and families with essential Community & Social Care services and resources. You will empower clients by guiding them through complex systems, helping them access the support they need to improve their lives. This position offers the unique opportunity to work directly within the community, building relationships and fostering positive change for residents in need.
Key Responsibilities
- Conduct needs assessments to identify individual and family requirements for social care and community support.
- Provide information, advice, and guidance on available services, resources, and entitlements.
- Assist clients in navigating and accessing relevant services, such as housing, health, and employment support.
- Develop and maintain strong relationships with local agencies, charities, and support groups.
- Facilitate referrals and coordinate with service providers to ensure seamless support for clients.
- Maintain accurate and up-to-date client records and case notes.
Requirements
- Experience in a social care, community work, or advisory role.
- Strong understanding of local services and resources available in the Wolverhampton area.
- Excellent communication, interpersonal, and listening skills.
- Ability to build trust and rapport with individuals from diverse backgrounds.
- Proficiency in IT, including case management systems and standard office software.
- A valid UK driving licence and access to a vehicle is essential for this role.
