Key Responsibilities

• Facilitate connections between service users and appropriate organisations and providers.
• Conduct virtual needs assessments and develop personalised action plans.
• Maintain accurate and confidential electronic records of client interactions and progress.
• Collaborate with community partners and service providers to enhance resource accessibility.

Requirements

• Experience in a social care, community development, or advisory role.
• Strong understanding of local community resources and support networks in the Southampton region.
• Excellent communication and active listening skills, adapted for remote interaction.
• Proficiency with virtual communication platforms and digital record-keeping systems.
• Ability to work independently and manage caseloads effectively from a remote setting.
• A proactive approach to problem-solving and resource identification.
