Community Nurse: Lead Compassionate Home-Based Care

Gloucestershire Health and Care NHS Foundation Trust (GHC) is seeking a Community Nurse to join the Integrated Community Team, delivering care to patients in their homes and clinic settings.

You will provide high-quality nursing care, assess needs, develop care plans with patients and carers, and support the ICT in managing caseloads.

This role offers opportunities to lead in absence of the Team Leader and to contribute to a person-centred approach across Gloucestershire.
