Community OT: Flexible Home Rehab & Discharge

South Warwickshire NHS Foundation Trust is seeking an enthusiastic Band 5 Occupational Therapist to join our Community Response Service, focusing on supporting hospital discharges and urgent therapy needs. You will work with patients in their own homes, coordinating with Community Nursing, GPs and acute care to deliver integrated rehabilitation across the pathway.

The role requires driving across Warwickshire with a full UK licence; weekend and bank holiday work is included.
