About the Role

Key Responsibilities

• Conduct proactive outreach to identify individuals and families in need of social care support within the designated community.
• Build and maintain strong relationships with clients, community leaders, and local organisations.
• Provide information and advice on available social care services, benefits, and support schemes.
• Assist clients in navigating complex systems and accessing relevant resources, overcoming barriers to engagement.
• Organise and facilitate community-based information sessions and support groups (online or in-person as appropriate).
• Maintain accurate records of outreach activities, client interactions, and referrals made.

Requirements

• Previous experience in community work, social care, or a related field is essential.
