About the Role
Our client is seeking a proactive and caring Community Support Assistant to join their team, primarily working remotely to support individuals across the Cambridge area. This role is crucial in ensuring that vulnerable individuals receive the necessary support to live independently within their own homes and communities. You will coordinate care packages, liaise with various professionals, and ensure that our clients' needs are met effectively and compassionately. This position offers the flexibility of remote working while maintaining a strong connection to the community services in Cambridgeshire .
Key Responsibilities
- Coordinate and manage daily care routines and support activities for assigned clients.
- Conduct initial assessments and ongoing reviews of client needs, developing and updating care plans accordingly.
- Liaise with families, healthcare providers, and social services to ensure a seamless and integrated support system.
- Monitor client progress and well-being, identifying potential issues and implementing proactive solutions.
- Maintain accurate and confidential records of client interactions, care provided, and any incidents.
- Provide a first point of contact for clients and their families, addressing concerns and ensuring satisfaction.
Requirements
- Previous experience in a social care or support role is essential, preferably with some coordination responsibilities.
- A strong understanding of person-centred care principles and safeguarding vulnerable adults.
- Excellent communication, organisational, and problem-solving skills.
- Proficiency in using digital record-keeping systems and standard office software.
- Ability to work autonomously and manage your workload effectively in a remote setting.
