Community Support Facilitator

About the Role

Our client is seeking a proactive and resourceful Community Support Facilitator to join their team in Sheffield . This role is focused on connecting individuals with the community resources and support networks they need to thrive. You will work directly with clients to identify their needs, build confidence, and empower them to engage more fully in community life. This position requires strong interpersonal skills, a passion for community development, and the ability to foster independence and well-being through practical support and guidance.

Key Responsibilities

• Engage with clients to identify their social, practical, and personal support needs.
• Facilitate access to local community groups, activities, and services.
• Support clients in developing skills and confidence to manage their daily lives.
• Build and maintain positive relationships with community organizations and service providers.
• Assist clients in overcoming barriers to social inclusion and participation.
• Maintain clear and accurate records of client interactions and progress.

Requirements

• Experience in a social care, community work, or support role is advantageous.
• Good understanding of community resources and support services in Sheffield .
• Excellent communication and interpersonal skills, with the ability to build rapport.
• A patient, empathetic, and resourceful approach.
• Ability to work independently and manage a small caseload.
• Full UK driving license and access to a vehicle would be beneficial for outreach work.
