Key Responsibilities
- Conduct comprehensive needs assessments for individuals seeking support, developing personalised care plans.
- Coordinate and deliver a range of support services, ensuring they meet the identified needs and preferences of clients.
- Liaise effectively with families, local authorities, healthcare providers, and other support agencies to ensure integrated care.
- Monitor the effectiveness of care plans, making adjustments as necessary and seeking feedback from clients.
- Provide emotional support and practical assistance to individuals and their families during challenging times.
- Maintain detailed and accurate records of all interactions, assessments, and care provision in line with organisational policies.
Requirements
- Proven experience in a social care or support role, with a good understanding of assessment frameworks.
- Strong knowledge of safeguarding procedures and the legal frameworks governing social care.
- Excellent organisational and time-management skills, with the ability to manage a caseload effectively.
- Effective communication and negotiation skills, able to build positive relationships with diverse individuals.
- Commitment to promoting independence, dignity, and choice for all service users.
- Relevant qualifications, such as NVQ Level 3 in Health and Social Care, or equivalent, are advantageous.
