About the Role
Our client is seeking a proactive and organised Community Support Officer to join their remote team. In this role, you will be instrumental in coordinating and delivering essential support services to individuals across various communities, utilising digital tools and platforms to connect with clients and stakeholders. You will play a key part in assessing needs, developing support strategies, and facilitating access to resources, all from a remote working setup. Key Responsibilities
- Conduct needs assessments and develop tailored support plans for individuals remotely.
- Coordinate and facilitate the delivery of community support services via digital communication channels.
- Liaise with external agencies, service providers, and stakeholders to ensure comprehensive client support.
- Maintain accurate and up-to-date records of client interactions, progress, and outcomes using online databases.
- Provide guidance and information to clients on available resources and services.
- Respond to client inquiries and concerns in a timely and professional manner via email, phone, or video conferencing.
Requirements
- Proven experience in community work, social care, or a related field.
- Strong understanding of support services and available resources for various client groups.
- Excellent verbal and written communication skills, with the ability to engage effectively with clients and colleagues online.
- Proficiency in using digital communication tools, case management software, and Microsoft Office Suite.
- Ability to work autonomously, manage workload effectively, and maintain motivation in a remote setting.
