Key Responsibilities

- Monitor client progress and adjust support strategies as needed, maintaining accurate case notes.
- Liaise with external agencies, charities, and local authorities to coordinate care and resources.
- Organize and lead group activities and workshops to promote social inclusion and skill development.
Requirements
- Proven experience in a social support or community development role.
- Strong understanding of social issues affecting vulnerable populations.
- Excellent assessment, planning, and problem-solving skills.
- Ability to build and maintain professional relationships with clients and stakeholders.
