About the Role
Our client is seeking an experienced and motivated Community Support Officer to join their team in Wolverhampton . This role involves providing a range of support services to individuals and families facing various challenges, helping them to access resources and improve their quality of life. You will play a key part in coordinating care plans, offering practical assistance, and acting as a link between individuals and community services. This position requires a proactive approach and the ability to work effectively both independently and as part of a team within the West Midlands region, with a blend of field and office-based responsibilities.
Key Responsibilities
- Conduct assessments of client needs and develop tailored support plans to address social, emotional, and practical challenges.
- Liaise with statutory and voluntary sector organizations to facilitate referrals and ensure integrated service delivery.
- Provide direct support and guidance to clients, empowering them to achieve their personal goals and increase independence.
- Maintain accurate and confidential records of client interactions, progress, and outcomes.
- Organize and facilitate group activities or workshops aimed at enhancing community engagement and well-being.
- Respond to crisis situations, providing immediate support and coordinating appropriate interventions.
Requirements
- Proven experience in a social care, community development, or support work role.
- Strong understanding of relevant legislation, policies, and best practices in social care.
- Excellent assessment, planning, and communication skills, with the ability to engage diverse client groups.
- Proficiency in using IT systems for record-keeping and reporting.
- Ability to travel within the Wolverhampton area, often requiring independent working.
