About the Role
Our client seeks a proactive and empathetic Community Support Officer to join their team in Coventry . This role focuses on empowering individuals facing various challenges, including mental health issues, social isolation, and difficulties accessing essential services. You will be instrumental in helping clients navigate complex systems, build essential life skills, and foster greater independence. Working within a multidisciplinary team, you will contribute to developing personalized support plans and facilitating access to community resources, fostering a supportive ecosystem for vulnerable individuals in the West Midlands area, promoting resilience and social integration.
Key Responsibilities
- Conduct needs assessments and develop tailored support plans for individuals within the community.
- Liaise with external agencies, including healthcare providers, local authorities, and voluntary organizations, to coordinate support services.
- Provide practical assistance and emotional support to clients, helping them to access benefits, housing, and employment opportunities.
- Organize and facilitate group activities and workshops aimed at improving social skills and community engagement.
- Maintain accurate and confidential client records and prepare regular progress reports.
- Advocate on behalf of clients to ensure their rights and needs are met.
Requirements
- Proven experience in a similar role within social care or a related field.
- Strong understanding of mental health, addiction, or homelessness issues.
- Excellent case management and reporting skills.
- Ability to work flexibly, including some evening and weekend work as required by the service needs.
- Proficiency in IT skills, including Microsoft Office and client management systems.
- A genuine passion for supporting vulnerable individuals and promoting their independence.
