About the Role

Our client is seeking a proactive and empathetic Community Support Officer to join their team based in Coventry . Key Responsibilities

• Conduct outreach activities to identify individuals and families requiring support within the Coventry area.
• Develop and implement tailored support plans, linking clients to relevant services and resources.
• Provide guidance, advocacy, and practical assistance to clients navigating complex social systems.
• Maintain detailed case notes and client records, ensuring confidentiality and accuracy.
• Collaborate with local charities, health services, and governmental agencies to coordinate care.
• Undertake administrative tasks and reporting from a remote setting as required by the role.

Requirements

• Experience in a similar community support or social services role.
• Strong understanding of local support networks and available resources in the West Midlands .
• Excellent communication, negotiation, and problem-solving skills.
• Ability to work effectively both independently in the field and remotely.
• Proficiency in standard office software and case management systems.
