About the Role
Our client is seeking a motivated and proactive Community Support Officer to join their team based in Belfast . This role focuses on empowering individuals within the community by providing tailored support to help them achieve their personal goals and enhance their quality of life. You will work collaboratively with service users, their families, and other agencies to develop and implement effective support plans, fostering independence and social inclusion. This position offers a dynamic and varied workload, contributing significantly to the positive outcomes for those we support in Belfast and surrounding areas.
Key Responsibilities
- Conduct needs assessments and develop personalised support plans for individuals.
- Provide practical and emotional support to service users, enabling them to live independently.
- Liaise with statutory and voluntary organisations to coordinate services and resources.
- Support individuals to access community facilities, employment, and social opportunities.
- Maintain accurate and confidential records of all interactions and progress.
- Advocate for service users' rights and needs.
Requirements
- Experience working within the community or social care sector.
- Strong understanding of support planning, risk assessment, and safeguarding procedures.
- Excellent communication, negotiation, and problem-solving skills.
- Ability to manage a caseload effectively and work autonomously.
- Proficiency in using IT systems for record-keeping and reporting.
