About the Role

Key Responsibilities

- Provide emotional support, encouragement, and practical assistance via phone, video calls, and secure messaging platforms.
- Assist clients in accessing community resources, services, and social opportunities, facilitating virtual engagement where possible.
- Monitor client progress, maintain accurate records, and report on outcomes and any emerging concerns to the management team.
- Facilitate online group activities and social interactions to combat isolation and promote community building among clients.
Requirements
- Proven experience in a similar community support, care, or social work role.
- Excellent communication, active listening, and empathy skills, with the ability to build rapport remotely.
- Proficiency in using digital communication tools, video conferencing software, and electronic record-keeping systems.
- A strong understanding of safeguarding principles and confidentiality protocols within the care sector.
- Ability to work independently, manage time effectively, and maintain professional boundaries in a remote setting.
