Key Responsibilities

• Provide personalized one-to-one support to clients in their own homes and within the local community, addressing needs such as personal care, mobility, and daily living activities.
• Assist clients with meal preparation, medication reminders, and attending appointments, ensuring their health and safety are prioritized.
• Encourage and facilitate social engagement, promoting independence and reducing isolation through planned activities and outings.
• Maintain accurate and confidential client records, documenting progress, incidents, and changes in care plans.
• Collaborate effectively with families, carers, and other healthcare professionals to ensure a coordinated approach to client care.
• Uphold the dignity and respect of all clients, adapting support strategies to meet their evolving needs and preferences.

Requirements

• Previous experience in a similar care or support role is highly desirable but not essential; a strong willingness to learn and a caring attitude are paramount.
• Excellent communication and interpersonal skills, with the ability to build rapport and trust with clients and their families.
• A valid driving license and access to a vehicle may be required for community-based tasks.
• Understanding of safeguarding principles and a commitment to promoting the welfare of vulnerable individuals.
• Ability to work flexibly, including evenings, weekends, and public holidays as per the rota.
• Proficiency in basic record-keeping and reporting.
