Key Responsibilities
- Provide direct personal care and support to clients in their own homes, assisting with daily living activities such as bathing, dressing, and meal preparation.
- Develop and implement individualized support plans in collaboration with clients, families, and case managers, regularly reviewing and updating these plans as needs change.
- Assist clients with medication management, ensuring adherence to prescribed dosages and schedules, and reporting any concerns to the nursing or management team.
- Encourage and facilitate social interaction and community engagement, accompanying clients to appointments, activities, or social events as required.
- Maintain accurate and confidential records of care provided, including observations, interventions, and client progress, using the company's designated reporting systems.
- Communicate effectively and professionally with clients, their families, and other members of the support network, fostering positive relationships and ensuring seamless care coordination.
Requirements
- Proven experience in a similar caregiving or support role, demonstrating empathy and a genuine desire to help others.
- Understanding of safeguarding principles and vulnerable adult protection procedures.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse individuals.
- Ability to work independently, manage time effectively, and respond calmly and professionally to challenging situations.
- Basic IT literacy for record-keeping and communication purposes.
- Flexibility to work a rotating schedule, including evenings and weekends, as per the needs of the service in London .
