Key Responsibilities

• Deliver personalized care and support according to individual care plans, ensuring client dignity and respect at all times.

• Accompany clients to appointments, social outings, and community activities, fostering engagement and reducing social isolation.
• Monitor and report any changes in a client's condition or well-being to the care manager promptly and accurately.
• Maintain clear and concise records of care provided, including observations and outcomes.
• Build positive and trusting relationships with clients, their families, and other healthcare professionals involved in their care.

Requirements

• Previous experience in a caregiving or support role is highly advantageous but not essential, as comprehensive training will be provided.
• Demonstrated empathy, patience, and a genuine passion for helping others.
• Excellent communication and interpersonal skills, with the ability to connect effectively with diverse individuals.
• Reliability, punctuality, and a strong sense of responsibility.
• Ability to work independently and as part of a supportive team.
• A basic understanding of safeguarding principles and confidentiality.
