About the Role
Our client is seeking a highly skilled and dedicated Complex Care Support Worker to provide essential support to individuals with complex health and social care needs, predominantly in the Reading area. This role is designed to be largely remote, focusing on care planning, coordination, and remote client support, with occasional necessary home visits. You will work closely with healthcare professionals to deliver tailored care packages, ensuring clients can maintain independence and quality of life within their own homes. Key Responsibilities
- Develop and implement detailed care plans for clients with complex needs, including medical conditions, disabilities, and behavioural challenges.
- Provide direct support to clients, including personal care, medication management, and therapeutic interventions.
- Monitor clients' health and well-being, accurately documenting observations and escalating concerns.
- Coordinate with families, medical teams, and other agencies to ensure seamless care delivery.
- Train and support junior care staff in the provision of complex care.
- Manage administrative duties related to client care, including scheduling and record-keeping via digital platforms.
Requirements
- Significant experience as a Support Worker, particularly with complex or long-term conditions.
- Proficiency in advanced care techniques and understanding of relevant medical conditions.
- Excellent communication, problem-solving, and decision-making skills.
- Ability to work autonomously and manage time effectively in a remote and field-based capacity.
- Relevant qualifications (e.g., NVQ Level 3/4 in Health and Social Care, specific clinical skills training).
- Valid driving licence and access to a reliable vehicle.
