About the Role
Our client is seeking a highly skilled and dedicated Complex Needs Support Worker for their domiciliary care services in Sheffield . This role is primarily remote, focusing on client assessment, care planning, and coordination, with direct client interaction occurring in their homes. You will be responsible for developing and overseeing personalised care packages for individuals with complex health conditions, disabilities, or challenging behaviours, ensuring they receive the highest quality of support to live independently. This is a vital role requiring a proactive, organised, and empathetic approach, ensuring client well-being and safety are paramount, within a supportive remote working structure.
Key Responsibilities
- Conduct thorough assessments of clients' complex care needs, including physical, psychological, and social requirements.
- Develop, implement, and regularly review comprehensive, person-centred care plans in collaboration with clients, families, and healthcare professionals.
- Coordinate the delivery of complex care services, ensuring appropriate staffing levels and skills are matched to client needs.
- Provide guidance and support to a team of remote and field-based care staff, offering supervision and training as needed.
- Ensure adherence to all relevant health and safety regulations, CQC standards, and best practices in complex care.
- Act as a key point of contact for clients, families, and external agencies, fostering strong collaborative relationships within the South Yorkshire area.
Requirements
- Significant experience in a support role, with a proven track record in managing complex care needs, such as spinal cord injuries, acquired brain injuries, or significant physical disabilities.
- In-depth knowledge of complex care principles, including catheter care, stoma care, PEG feeding, and palliative care.
- Excellent assessment, care planning, and communication skills, with the ability to manage challenging situations sensitively.
- Proficiency in using digital platforms for client management, communication, and reporting, essential for remote operations.
- A recognised qualification in health and social care, such as a QCF/NVQ Level 3 or above, or equivalent clinical experience.
- A proactive and adaptable approach, demonstrating a commitment to promoting independence and quality of life for clients in Sheffield .
