Role Summary

The Care Coordination Pharmacist will play a crucial role in ensuring the safe and effective use of medications within the Care Coordination team. This position involves conducting clinical medication reviews, providing expert advice on drug interactions and adverse effects, and working closely with a multidisciplinary team to optimize patient care.

Key Responsibilities

Clinical Practice

• Medication Reviews: Conduct thorough clinical medication reviews for patients requiring additional support.
• Prescribing Recommendations: Make informed prescribing recommendations and communicate these decisions to all relevant stakeholders.
• Safety and Compliance: Ensure the safe use of medicines by regularly reviewing prescriptions, identifying drug interactions, and monitoring for adverse effects. Ensure compliance with legislation such as the Medicines Act and the Mental Health Act, as well as Trust policies and guidelines.
• Clinical Pharmacy Services: Provide a comprehensive clinical pharmacy service to the Care Coordination team, assigned wards, and the multidisciplinary team involved in patient care.
• Team Support: Actively participate in the pharmacy and medicines management team, offering support and acting as a clinical role model for less experienced staff.
• Expert Consultation: Seek advice from senior pharmacy colleagues for guidance on unlicensed medicines use and areas with limited evidence base. Provide expert advice to clinicians on complex medicine-related issues.

Multidisciplinary Collaboration

• Team Integration: Work as part of a multidisciplinary team to ensure safe, clinically effective, and cost-efficient use of medicines. Participate in consultant ward rounds and multidisciplinary team meetings.
• Specialized Advice: Offer highly specific advice on managing complex issues such as drug interactions, medication use during pregnancy, and unlicensed medicine use.
• Referrals: Make appropriate referrals to the Lead Pharmacist SEECHS, other multidisciplinary team members, and within the pharmacy team.

Professional Guidance

• Educational Role: Advise medical, dental, nursing staff, and allied healthcare professionals on the actions and safe, cost-effective use of drugs. Critically appraise literature to provide accurate interpretations for improving medicine use.
• Medicines Information: Utilize medicines information resources to enhance clinical pharmacy practice in community health services.
• Independent Prescribing: Work as an independent non-medical prescriber within designated areas of practice. This may follow training and a period of supplementary prescribing as needed. Be responsible and accountable for patient assessments and clinical management decisions, including prescribing.
