Dementia Care Practitioner - Home Support

About the Role

Our client is seeking experienced and compassionate Dementia Care Practitioners to provide specialised home-based support in Norwich . This vital role focuses on assisting individuals living with dementia to maintain their independence, dignity, and quality of life within their own familiar environment. You will work closely with clients and their families, developing and implementing personalised care plans that address the unique challenges associated with dementia. Key Responsibilities

• Provide personalised care and support tailored to the needs of individuals with dementia, including personal care and daily living tasks.
• Develop and maintain positive, trusting relationships with clients and their families, offering emotional support and reassurance.
• Implement and review care plans, adapting approaches to meet evolving cognitive and physical needs.
• Assist with medication management, ensuring safety and adherence to protocols.
• Engage clients in meaningful activities that stimulate cognitive function and promote well-being.
• Liaise effectively with healthcare professionals, families, and the wider care team to ensure coordinated support.

Requirements

• Previous experience working as a Dementia Care Practitioner or in a similar role supporting individuals with cognitive impairments.
• A Level 2 or 3 Diploma in Health and Social Care is highly preferred.
• Strong understanding of dementia, its progression, and best practice care approaches.
• Excellent communication, empathy, and patience skills.
• Ability to work independently and manage time effectively within a home setting.
• Must possess a full UK driving licence and have access to a vehicle.
