About the Role
Our client is looking for a proactive and empathetic Disability Support Coordinator to join their vibrant team in Leeds, West Yorkshire . This role is dedicated to empowering individuals with disabilities to live more independent and fulfilling lives. You will play a crucial role in assessing needs, developing personalized support plans, and connecting individuals with essential resources and services within the community. Operating on a hybrid model, you will balance essential face-to-face interactions with efficient remote administrative tasks, ensuring seamless coordination of care. Key Responsibilities
- Conduct thorough needs assessments for individuals with various disabilities, identifying specific support requirements.
- Develop, implement, and regularly review personalized support plans in collaboration with clients and their families.
- Coordinate and arrange necessary support services, including personal care, community access, and therapeutic interventions.
- Liaise with healthcare professionals, social workers, and other relevant agencies to ensure integrated care.
- Provide information, advice, and advocacy to clients and their families regarding their rights and available services.
- Maintain accurate client records and prepare reports for funding bodies and internal reviews.
Requirements
- Previous experience in a similar role supporting individuals with disabilities.
- In-depth knowledge of disability services, relevant legislation, and best practice frameworks.
- Excellent communication, assessment, and care planning skills.
- Ability to manage a caseload effectively and work autonomously within a hybrid structure.
- A relevant qualification in Health and Social Care, Psychology, or a related field is highly desirable.
